High Fidelity Wraparound is a team-based approach that brings together the youth, family, and providers to create a personalized plan of care. It focuses on identifying the family’s strengths, addresses their unique needs, and provides coordinated support to help children succeed at home, in school, and in the community and help them reach their hopes and dreams.

 

People will make positive changes if they are honored for who they are, for the strengths they have, and if they get their underlying needs met.
